IMPLEMENTASI KEPERAWATAN LATIHAN BATUK EFEKTIF PADA PASIEN LANSIA TBC DENGAN MASALAH BERSIHAN JALAN NAFAS TIDAK EFEKTIF DI PUSKESMAS SUKARAMI LAHAT TAHUN 2025

Latar Belakang: Laporan Global TB Report 2022 mencatat bahwa Indonesia
termasuk negara dengan beban TBC tertinggi di dunia, dengan 969.000 kasus,
Penumpukan sputum akibat TBC pada saluran napas dapat menyebabkan obstruksi,
dan menyebabkan ketidakcukupan oksigenasi. Oleh karena itu, penting untuk
melakukan tindakan yang dapat memfasilitasi pengeluaran sputum, yakni latihan batuk
efektif. Batuk efektif dapat membantu membersihkan jalan napas dan mengurangi
penumpukan lendir.
Metode: Rencana studi kasus yang digunakan adalah metode deskriptif dengan
pendekatan proses asuhan keperawatan yang meliputi tahap pengkajian, diagnosis
keperawatan, intervensi keperawatan, implementasi keperawatan, dan evaluasi
keperawatan. Metode pengumpulan data menggunakan wawancara, observasi,
pemeriksaan fisik dan studi dokumentasi. Penelitian ini dilakukan terhadap dua orang
pasien dengan diagnosa medis Tuberculosis Paru dengan masalah keperawatan
bersihan jalan nafas tidak efektif di Puskesmas Sukarami Lahat Tahun 2025. Studi
kasus dilakukan selama 3 hari yaitu kepada pasien 1 tanggal 24 Februari-26 Februari
2025 dan Pasien 2 tanggal 27 Februari-01 Maret 2025.
Hasil: Setelah dilakukan implementasi Latihan batuk efektif selama 3 hari pada pasien
I (Ny. T) terjadi perbaikan kondisi dengan kriteria hasil batuk efektif meningkat,
produksi sputum cukup menurun, ronkhi cukup menurun, dispnea menurun, frekuensi
napas cukup membaik dan pola napas cukup membaik. Sedangkan pada pasien II (Tn.
M) terjadi perbaikan kondisi dengan batuk efektif cukup meningkat, produksi sputum
cukup menurun, ronkhi sedang, dispnea cukup menurun, gelisah menurun, frekuensi
napas cukup membaik dan pola napas cukup membaik.
Kesimpulan: Implementasi keperawatan latihan batuk efektif terbukti mampu
meningkatkan bersihan jalan napas pada pasien lansia dengan TBC.

Background: The Global TB Report 2022 states that Indonesia is among the countries
with the highest tuberculosis burden in the world, with 969,000 reported cases. The
accumulation of sputum in the airways due to tuberculosis can lead to obstruction and
impaired oxygenation. Therefore, interventions that facilitate sputum clearance, such
as effective coughing exercises, are essential. Effective coughing helps clear the airway
and reduces mucus buildup.
Methods: This case study employed a descriptive method with a nursing care process
approach, which included assessment, nursing diagnosis, nursing intervention, nursing
implementation, and evaluation. Data collection methods included interviews,
observation, physical examination, and documentation review. The study involved two
patients diagnosed with pulmonary tuberculosis and experiencing ineffective airway
clearance at Puskesmas Sukarami Lahat in 2025. The case study was conducted over
three days: Patient 1 from February 24 to February 26, 2025, and Patient 2 from
February 27 to March 1, 2025.
Results: Following the implementation of effective coughing exercises for three
consecutive days, Patient I (Mrs. T) showed improvement marked by increased
effective coughing, reduced sputum production, decreased rhonchi sounds, reduced
dyspnea, and improved respiratory rate and breathing pattern. Similarly, Patient II (Mr.
M) demonstrated moderate improvements in effective coughing, reduced sputum
production, decreased rhonchi, reduced dyspnea and restlessness, and improved
respiratory rate and breathing pattern.
Conclusion: The implementation of effective coughing exercises in nursing care has
been proven to improve airway clearance in elderly patients with tuberculosis.
