IMPLEMENTASI KEPERAWATAN FISIOTERAPI DADA PERKUSI (CLAPPING) PADA ANGGOTA KELUARGA PENDERITA TUBERKULOSIS DENGAN MASALAH BERSIHAN JALAN NAFAS TIDAK EFEKTIF DIWILAYAH KERJA PUSKESMAS SELAWI TAHUN 2025

Latar Belakang: Tuberkulosis paru merupakan salah satu penyakit
menular yang prevalensinya tinggi di Indonesia. Salah satu masalah
keperawatan yang sering dialami oleh penderita TB paru adalah bersihan
jalan napas tidak efektif akibat penumpukan sputum. Fisioterapi dada,

khususnya teknik perkusi (clapping), merupakan intervensi non-
farmakologis yang efektif dalam membantu pengeluaran sputum dan

memperbaiki ventilasi paru.
Metode: Penelitian ini merupakan studi kasus dengan pendekatan asuhan
keperawatan keluarga. Subjek studi adalah dua klien dengan TB paru dan
masalah bersihan jalan napas tidak efektif. Data dikumpulkan melalui
wawancara, observasi, pemeriksaan fisik, serta dokumentasi implementasi
fisioterapi dada. Intervensi dilakukan selama tiga hari berturut-turut dan
dievaluasi berdasarkan peningkatan efektivitas bersihan jalan napas.
Hasil: Setelah dilakukan fisioterapi dada metode clapping, terjadi
peningkatan efektivitas bersihan jalan napas yang ditandai dengan batuk
yang lebih efektif, penurunan jumlah sputum, tidak terdengarnya ronki,
serta frekuensi napas yang membaik pada kedua subjek.
Kesimpulan: Fisioterapi dada metode clapping merupakan intervensi
yang efektif dalam meningkatkan efektivitas bersihan jalan napas pada
pasien TB paru. Intervensi ini dapat dilakukan secara mandiri oleh
keluarga sebagai bagian dari asuhan keperawatan keluarga.

Background: Pulmonary tuberculosis is one of the infectious diseases
with a high prevalence in Indonesia. One of the nursing problems that is
often experienced by pulmonary TB patients is ineffective airway cleaning
due to sputum buildup. Chest physiotherapy, especially percussion
techniques (clapping), is an effective non-pharmacological intervention in
helping sputum removal and improving lung ventilation.
Methods: This study is a case study with a family nursing care approach.
The study subjects were two clients with pulmonary TB and the airway
clearance problem was ineffective. Data were collected through
interviews, observations, physical examinations, and documentation of the
implementation of chest physiotherapy. The intervention was carried out
for three consecutive days and was evaluated based on the increased
effectiveness of airway clearance.
Results: After chest physiotherapy of the clapping method, there was an
increase in the effectiveness of airway clearance which was characterized
by a more effective cough, a decrease in the amount of sputum, no sound
of ronki, and improved breathing frequency in both subjects.
Conclusion: Chest physiotherapy clapping method is an effective
intervention in increasing the effectiveness of airway clearance in
pulmonary TB patients. This intervention can be carried out independently
by the family as part of family nursing care.
