Abstract

Abstrak
Latar Belakang: Kanker serviks merupakan salah satu penyebab utama morbiditas dan
mortalitas pada wanita yang sebagian besar berhubungan dengan infeksi Human
Papillomavirus (HPV) risiko tinggi. Selain HPV, faktor usia dan tingkat pendidikan diduga
berperan dalam meningkatkan risiko terjadinya kanker serviks. Identifikasi faktor risiko
berbasis data lokal diperlukan untuk mendukung upaya pencegahan dan deteksi dini kanker
serviks. Tujuan: Menganalisis hubungan usia, status HPV, dan tingkat pendidikan dengan
kejadian kanker serviks di RSUD Siti Fatimah Provinsi Sumatera Selatan Tahun 2025.
Metode: Penelitian ini menggunakan desain observasional analitik dengan pendekatan
case-control. Sampel berjumlah 84 responden yang terdiri dari 42 kelompok kasus (kanker
serviks) dan 42 kelompok kontrol (bukan kanker serviks) yang dipilih menggunakan
stratified random sampling berdasarkan data rekam medis tahun 2025. Variabel independen
meliputi usia, status HPV, dan tingkat pendidikan, sedangkan variabel dependen adalah
kejadian kanker serviks. Analisis data dilakukan secara univariat dan bivariat
menggunakan uji chi-square/fisher exact test dengan tingkat kemaknaan α = 0,05 serta
perhitungan Odds Ratio (OR). Hasil: Sebagian besar penderita kanker serviks berusia ≥35
tahun (90,5%), dinyatakan HPV positif (64,3%), dan berpendidikan ≤SMP (71,4%).
Analisis bivariat menunjukkan terdapat hubungan yang signifikan antara usia dengan
kejadian kanker serviks (p=0,02; OR=4,259; 95%CI=1,257–14,431), sehingga wanita usia
≥35 tahun memiliki risiko 4,259 kali lebih besar mengalami kanker serviks dibandingkan
wanita usia ≤35 tahun. Status HPV juga berhubungan signifikan dengan kejadian kanker
serviks (p<0,001), di mana seluruh responden dengan HPV positif ditemukan pada
kelompok kanker serviks. Selain itu, tingkat pendidikan berhubungan signifikan dengan
kejadian kanker serviks (p=0,02), dengan kejadian kanker serviks lebih banyak ditemukan
pada wanita berpendidikan ≤SMP dibandingkan wanita berpendidikan ≥SMA.
Kesimpulan: Usia ≥35 tahun, infeksi HPV, dan tingkat pendidikan rendah berhubungan
secara signifikan dengan kejadian kanker serviks di RSUD Siti Fatimah Provinsi Sumatera
Selatan. Infeksi HPV merupakan faktor yang menunjukkan hubungan paling kuat dengan
kejadian kanker serviks. Peningkatan cakupan vaksinasi HPV, edukasi kesehatan
reproduksi, serta pelaksanaan skrining rutin melalui IVA, pap smear, dan pemeriksaan
HPV DNAperlu ditingkatkan sebagai upaya pencegahan kanker serviks.

Abstract
Background:Cervical cancer remainsoneoftheleadingcauses of morbidity andmortality
among women worldwide and is strongly associated with high-risk Human Papillomavirus
(HPV) infection. In addition to HPV infection, age and educational level are considered
important determinants of cervical cancer risk. Identification of local risk factors is
essential to support cervical cancer prevention and early detection strategies. Objective:
To analyze the association between age, HPV status, and educational level with the
incidence of cervical cancer at Siti Fatimah Regional General Hospital, South Sumatra
Province, Indonesia, in 2025. Methods: This analytical observational study employed a
case-control design. A total of 84 respondents were included, consisting of 42 cervical
cancer cases and 42 controls selected using stratified random sampling from medical
record data in 2025. Independent variables included age, HPV status, and educational
level, while the dependent variable was cervical cancer incidence. Data were analyzed
using univariate and bivariate analyses with Chi-square/Fisher's Exact Test at a
significance level of α = 0.05, followed by Odds Ratio (OR) estimation. Results: Most
cervical cancer patients were aged ≥35 years (90.5%), HPV-positive (64.3%), and had an
educational level of ≤ junior high school (71.4%). Bivariate analysis revealed a significant
association between age and cervical cancer incidence (p=0.02; OR=4.259;
95%CI=1.257–14.431), indicating that women aged ≥35 years were 4.259 times more
likely to develop cervical cancer than those aged ≤35 years. HPV status was also
significantly associated with cervical cancer incidence (p<0.001), with all HPV-positive
respondents belonging to the cervical cancer group. Furthermore, educational level was
significantly associated with cervical cancer incidence (p=0.02), with cervical cancer
occurring more frequently among women with lower educational attainment (≤ junior high
school). Conclusion: Age ≥35 years, HPV infection, and low educational level were
significantly associated with cervical cancer incidence at Siti Fatimah Regional General
Hospital, South Sumatra Province. HPV infection demonstrated the strongest association
with cervical cancer occurrence. Strengthening HPV vaccination coverage, reproductive
health education, and routine screening programs through VIA, Pap smear, and HPV DNA
testing is recommended to reduce the burden of cervical cancer.
