Abstract
Latar belakang: High Alert Medication (HAM) merupakan kelompok obat yang memiliki risiko tinggi menyebabkan cedera serius atau bahkan kematian apabila terjadi kesalahan dalam proses pengelolaannya. Penyimpanan yang tidak sesuai standar dapat meningkatkan risiko terjadinya medication error dan berdampak pada keselamatan pasien. Penelitian ini bertujuan untuk menegaskan kesesuaian gambaran penyimpanan obat High Alert Medication (HAM) di Instalasi Farmasi Rumah Sakit Umum Daerah (RSUD) Kayu Agung.
Metode: Jenis penelitian ini menggunakan pendekatan deskriptif dengan rancangan observasional pengumpulan data dilakukan melalui observasi langsung menggunakan lembar checklist yang mengacu pada Peraturan Menteri Kesehatan Republik Indonesia Nomor 72 Tahun 2016 tentang Standar Pelayanan Kefarmasian di Rumah Sakit. Objek penelitian meliputi area penyimpanan obat High Alert Medication di ruang rawat inap, rawat jalan, gudang farmasi, depo operasi (OK), dan Instalasi Gawat Darurat (IGD). Data dianalisis secara deskriptif menggunakan perhitungan persentase kesesuaian.
Hasil: Hasil penelitian menunjukkan bahwa seluruh indikator penyimpanan obat High Alert dan LASA pada lima unit penyimpanan telah memenuhi standar yang ditetapkan. Sebanyak 13 indikator observasi memperoleh hasil sesuai pada seluruh unit dengan persentase kesesuaian masing-masing sebesar 100%. Kesesuaian tersebut meliputi aspek pelabelan, pemisahan penyimpanan, pengaturan suhu, penerapan sistem FEFO, penyimpanan berdasarkan jenis sediaan, serta pengamanan obat narkotika dan psikotropika.
Kesimpulan: Penyimpanan obat High Alert Medication (HAM) di Instalasi Farmasi RSUD Kayu Agung telah sesuai dengan standar yang berlaku dan mendukung upaya peningkatan keselamatan pasien serta mutu pelayanan kefarmasian di rumah sakit.

Background: High Alert Medications (HAM) are a group of drugs that carry a high risk of causing serious injury or even death when errors occur during their management. Improper storage practices may increase the risk of medication errors and negatively affect patient safety. This study aimed to determine the compliance of High Alert Medication (HAM) storage practices at the Pharmacy Installation of Regional General Hospital of Kayu Agung with established standards
Methods: This study employed a descriptive observational approach. Data were collected through direct observation using a checklist based on the Regulation of the Minister of Health of the Republic of Indonesia Number 72 of 2016 concerning Standards of Pharmaceutical Services in Hospitals. The observation covered High Alert Medication storage areas in inpatient units, outpatient units, the pharmacy warehouse, the operating room pharmacy depot, and the Emergency Department (ED). Data were analyzed descriptively using percentage calculations of compliance.
Results: The results showed that all indicators related to the storage of High Alert Medications and Look-Alike Sound-Alike (LASA) drugs in the five storage units met the established standards. All 13 observation indicators were found to be compliant in every unit, resulting in a compliance rate of 100%. Compliance included labeling, storage separation, temperature control, implementation of the First Expired First Out (FEFO) system, storage based on dosage forms, and secure storage of narcotic and psychotropic medications.
Conclusion: The storage of High Alert Medications (HAM) in the Pharmacy Installation of Kayu Agung Regional General Hospital was fully compliant with applicable standards and supports efforts to improve patient safety and the quality of pharmaceutical services.
