Abstract

Latar Belakang: Kemoterapi merupakan salah satu terapi utama pada pasien kanker

yang dapat menimbulkan efek samping berupa mual dan muntah atau Chemotherapy-Induced Nausea and Vomiting (CINV). Kondisi tersebut dapat menurunkan kualitas hidup pasien sehingga diperlukan penggunaan antiemetik sesuai dengan tingkat risiko emetogenik obat kemoterapi. Penelitian ini bertujuan untuk menggambarkan penggunaan antiemetik pada pasien kemoterapi di RSUP Dr. Mohammad Hoesin Palembang berdasarkan tingkat risiko emetogenik obat kemoterapi.

Metode: Penelitian ini merupakan penelitian non-eksperimental dengan desain deskriptif observasional dan pendekatan retrospektif. Data diperoleh dari rekam medis pasien kanker yang menjalani kemoterapi di RSUP Dr. Mohammad Hoesin Palembang menggunakan teknik purposive sampling sesuai kriteria inklusi dan eksklusi. Data yang dikumpulkan meliputi karakteristik pasien, jenis kanker, tingkat emetogenik kemoterapi, dan penggunaan antiemetik berdasarkan pedoman National Comprehensive Cancer Network (NCCN).

Hasil: Hasil penelitian menunjukkan bahwa pasien kemoterapi didominasi oleh perempuan dengan kelompok usia dewasa hingga lanjut usia, serta diagnosis kanker yang paling banyak ditemukan adalah kanker mammae. Regimen kemoterapi yang digunakan sebagian besar termasuk dalam kategori risiko emetogenik rendah. Golongan antiemetik yang paling banyak digunakan adalah 5-HT3 receptor antagonist, terutama ondansetron, diikuti oleh golongan kortikosteroid. Penggunaan golongan antagonis dopamin dan antikolinergik/antihistamin ditemukan dalam jumlah yang lebih sedikit.

Kesimpulan: Penggunaan antiemetik pada pasien kanker yang menjalani kemoterapi di RSUP Dr. Mohammad Hoesin Palembang didominasi oleh golongan 5-HT3 Receptor Antagonist, diikuti kortikosteroid dan antagonis dopamin. Penggunaan golongan antiemetik tersebut menggambarkan bahwa terapi pencegahan mual dan muntah pada pasien kemoterapi telah disesuaikan dengan kebutuhan klinis sehingga diharapkan mampu mengurangi kejadian CINV dan meningkatkan kenyamanan pasien selama menjalani kemoterapi.

Background: Chemotherapy is one of the primary treatments for cancer patients and can cause side effects such as nausea and vomiting, known as Chemotherapy-Induced Nausea and Vomiting (CINV). These symptoms can reduce a patient's quality of life, making it necessary to use antiemetics appropriate for the emetogenic risk level of the chemotherapy drug. This study aims to describe the use of antiemetics in chemotherapy patients at Dr. Mohammad Hoesin General Hospital in Palembang based on the emetogenic risk level of the chemotherapy drugs.

Methods: This study is a non-experimental study with a descriptive observational design and a retrospective approach. Data were obtained from the medical records of cancer patients undergoing chemotherapy at Dr. Mohammad Hoesin General Hospital in Palembang using purposive sampling according to inclusion and exclusion criteria. The data collected included patient characteristics, cancer type, the emetogenicity level of chemotherapy, and the use of antiemetics based on the National Comprehensive Cancer Network (NCCN) guidelines.

Results: Research findings indicate that the use of antiemetics in chemotherapy patients varies depending on the emetogenicity of the chemotherapy regimen. The most commonly used classes of antiemetics are 5-HT3 antagonists and corticosteroids. Appropriate use of antiemetics can help reduce the incidence of nausea and vomiting in cancer patients undergoing chemotherapy.

Conclusion: The study results show that chemotherapy patients are predominantly women in the adult to elderly age groups, and the most common cancer diagnosis is breast cancer. The chemotherapy regimens used mostly fall into the low emetogenic risk category. The most commonly used class of antiemetics is 5-HT3 receptor antagonists, particularly ondansetron, followed by corticosteroids. The use of dopamine antagonists and anticholinergics/antihistamines was found to be less common.
