Abstract

Latar Belakang : Penggunaan antibiotik yang tidak rasional pada pasien rawat inap dapat meningkatkan risiko resistensi bakteri, kegagalan terapi, serta memperburuk kualitas pelayanan kesehatan. Pasien penyakit dalam memiliki risiko mengalami infeksi karena adanya berbagai penyakit penyerta sehingga penggunaan antibiotik perlu dievaluasi untuk memastikan kesesuaiannya dengan pedoman terapi yang berlaku.

Metode : Penelitian ini merupakan penelitian deskriptif dengan pendekatan observasional prospektif yang dilakukan di ruang rawat inap kelas 3 RSUD Siti Fatimah Palembang pada Februari–April 2026. Sampel penelitian sebanyak 124 pasien. Berdasarkan diagnosis utama, ditemukan 32 kasus infeksi dan 92 kasus non-infeksi. Evaluasi ketepatan penggunaan antibiotik difokuskan pada 32 kasus infeksi dengan 38 pemberian antibiotik. Data dikumpulkan dari rekam medis meliputi jenis kelamin, diagnosis penyakit, jenis antibiotik, dosis, interval, rute, lama penggunaan antibiotik.

Hasil : Hasil penelitian menunjukkan bahwa penggunaan antibiotik didominasi oleh golongan sefalosporin sebesar 67,9%, terutama ceftriaxone sebagai antibiotik yang paling banyak digunakan sebesar 56,2%. Pada 32 kasus infeksi dengan 38 pemberian antibiotik, diperoleh ketepatan pemberian antibiotik sebesar 55,2%, ketepatan dosis antibiotik sebesar 89,4%, dan ketepatan durasi antibiotik sebesar 18,4%. Ketidaktepatan terutama ditemukan pada aspek durasi terapi, yaitu sebesar 81,6%. Selain itu, ditemukan 92 kasus dengan diagnosis utama non-infeksi yang juga mendapatkan antibiotik, tetapi tidak dievaluasi lebih lanjut karena keterbatasan data klinis.

Kesimpulan : Penggunaan antibiotik pada pasien penyakit dalam di RSUD Siti Fatimah Palembang belum sepenuhnya rasional, terutama pada aspek durasi terapi.

Background : Irrational use of antibiotics in hospitalized patients can increase the risk of bacterial resistance, therapeutic failure, and worsen the quality of healthcare services. Internal medicine patients are at risk of infection due to various comorbidities, so antibiotic use needs to be evaluated to ensure its compliance with applicable therapeutic guidelines.

Methods : This is a descriptive study with a prospective observational approach conducted in the Class 3 inpatient ward of Siti Fatimah Regional Hospital, Palembang, from February to April 2026. The study sample consisted of 124 patients. Based on the primary diagnosis, 32 cases of infection and 92 cases of non-infection were found. The evaluation of antibiotic use appropriateness focused on 32 cases of infection, with 38 antibiotic administrations. Data collected from medical records included gender, disease diagnosis, antibiotic type, dose, interval, route, and duration of antibiotic use.

Results : The study showed that antibiotic use was dominated by cephalosporins at 67.9%, with ceftriaxone being the most commonly used antibiotic at 56.2%. In 32 infection cases with 38 antibiotic prescriptions, antibiotic administration accuracy was 55.2%, antibiotic dosage accuracy was 89.4%, and antibiotic duration accuracy was 18.4%. Inaccuracy was primarily found in the duration of therapy, at 81.6%. In addition, 92 cases with a primary diagnosis of a non-infection also received antibiotics, but were not further evaluated due to limited clinical data.

Conclusion : Antibiotic use in internal medicine patients at Siti Fatimah Regional Hospital, Palembang is not fully rational, especially regarding the duration of therapy.
