ASUHAN KEBIDANAN KOMPREHENSIF PADA NY ”Y” DI TEMPAT PRAKTIK KLINIK AULIA KHALIK MEDIKA KOTA PALEMBANG TAHUN 2026

Latar Belakang: Pendekatan Continuity of Care (CoC) sejak kehamilan hingga keluarga berencana (KB) penting untuk mendeteksi dini komplikasi dan menurunkan angka kematian ibu serta bayi.Tujuan: Memberikan asuhan kebidanan secara komprehensif pada Ny. "Y" menggunakan manajemen kebidanan terdokumentasi SOAP.Metode: Studi kasus deskriptif dengan pendekatan CoC pada Ny. "Y" (G3P2A0) dari usia kehamilan 33 minggu, persalinan, nifas, neonatus, hingga pelayanan KB. Data dikumpulkan melalui anamnesis, pemeriksaan fisik, observasi, dan dokumentasi.Hasil: Asuhan kehamilan berjalan normal dengan KIE keluhan fisiologis yang tepat. Persalinan berlangsung spontan pada usia 39 minggu (Kala I–IV normal); bayi laki-laki lahir sehat (BB 3.100 gram, PB 49 cm, Apgar 9/10) tanpa laserasi jalan lahir. Masa nifas dan involusi uterus normal, bayi mendapat imunisasi HB0, dan ibu memilih menggunakan metode KB pasca-salin.Kesimpulan: Asuhan komprehensif pada Ny. "Y" terlaksana dengan baik sesuai standar pelayanan kebidanan dan tidak ditemukan kesenjangan antara teori dan praktik.

Background: The Continuity of Care (CoC) approach from pregnancy to family planning is vital for early complication detection and reducing maternal and infant mortality.Objective: To provide comprehensive midwifery care for Mrs. "Y" using a midwifery management approach with SOAP documentation.Methods: A descriptive case study using a CoC approach on Mrs. "Y" (G3P2A0) from 33 weeks of gestation, labor, postpartum, neonatal periods, to family planning services. Data were collected through anamnesis, physical examinations, observation, and documentation.Results: Antenatal care progressed normally with appropriate education on physiological complaints. Delivery occurred spontaneously at 39 weeks (Stages I–IV were normal); a healthy baby boy was born (weight 3,100 g, length 49 cm, Apgar score 9/10) with no perineal lacerations. Postpartum care and uterine involution were normal, the infant received the HB0 immunization, and the mother chose to use a postpartum family planning method.Conclusion: Comprehensive care for Mrs. "Y" was successfully implemented in accordance with midwifery clinical standards, showing no gaps between theory and practice.
