ASUHAN KEBIDANAN KOMPREHENSIF PADA NY “S” DI PRAKTIK MANDIRI BIDAN TETI HERAWATI KOTA PALEMBANG TAHUN 2026

Latar Belakang : Asuhan Kebidanan Komprehensif merupakan asuhan yang diberikan secara menyeluruh dan berkesinambungan kepada ibu mulai dari masa kehamilan, persalinan, nifas, bayi baru lahir hingga pelayanan keluarga berencana melalui pendekatan Continuity Of Care (COC). Asuhan ini bertujuan untuk memantau kondisi ibu dan bayi serta mendeteksi secara dini kemungkinan terjadinya komplikasi sehingga dapat membantu menurunkan angka kematian ibu dan bayi.
Tujuan : Penyusunan laporan ini bertujuan untuk mendokumentasikan Asuhan Kebidanan Komprehensif yang telah dilakukan pada Ny. "S" di Tempat Praktik Mandiri Bidan Teti Herawati Kota Palembang Tahun 2026 dengan metode pendokumentasian SOAP. Laporan ini memuat studi kasus secara berkesinambungan mulai dari masa kehamilan trimester III, persalinan, nifas, bayi baru lahir, hingga pelayanan keluarga berencana.
Metode : Pengumpulan data pada laporan ini menggunakan teknik wawancara (anamnesa), observasi secara langsung, dan pemeriksaan fisik. Data yang digunakan meliputi data primer dan data sekunder yang diperoleh melalui hasil wawancara, analisis buku KIA, pemeriksaan fisik, serta pemeriksaan penunjang.
Hasil : Asuhan kehamilan berjalan normal dengan KIE keluhan fisiologis yang tepat. Persalinan berlangsung spontan pada usia 37 minggu (kala I-IV normal) ; bayi laki-laki sehat (BB 3.200 gram, PB 49 cm, Apgar 9/10) tanpa laserasi jalan lahir. Masa nifas dan involusi uterus normal, bayi mendapat imunisasi HB0, dan ibu memilih menggunakan metode KB pasca-salin.
Kesimpulan : Asuhan komprehensif pada Ny. “S” terlaksana dengan baik sesuai standar pelayanan kebidanan dan tidak ditemukan kesenjangan antara teori dan praktik.

Background : Comprehensive Midwifery Care is care provided holistically and continuously to mothers starting from pregnancy, childbirth, postpartum, newborn care, to family planning services through a Continuity of Care (CoC) approach. This care aims to monitor the condition of the mother and baby and to detect early potential complications, thereby helping to reduce maternal and infant mortality rates.
Objective : The purpose of this report is to document the Comprehensive Midwifery Care provided to Mrs. "S" at Midwife Teti Herawati’s Independent Practice, Palembang City, in 2026 using the SOAP documentation method. This report contains a continuous case study starting from the third trimester of pregnancy, childbirth, postpartum, newborn, to family planning services.
Methods : Data collection in this report used interview techniques (anamnesis), direct observation, and physical examinations. The data used include primary and secondary data obtained through interviews, Maternal and Child Health (KIA) book analysis, physical examinations, and diagnostic tests.
Results : Antenatal care proceeded normally with appropriate counseling on physiological complaints. Delivery occurred spontaneously at 37 weeks of gestation (with normal stages I-IV); a healthy baby boy was born (weight: 3,200 grams, length: 49 cm, Apgar score: 9/10) with no perineal lacerations. The postpartum period and uterine involution were normal, the infant received the HB0 (Hepatitis B) immunization, and the mother opted for a postpartum family planning method.
Conclusion : Comprehensive care for Mrs. "S" was successfully implemented in accordance with midwifery service standards, and no gaps were found between theory and clinical practice.
