Abstract

Latar Belakang: Pemeriksaan sitologi cairan pleura merupakan salah satu pemeriksaan penunjang yang penting dalam membantu dokter menegakkan diagnosis berbagai kelainan rongga pleura. Kualitas sediaan hasil pewarnaan sangat menentukan kejelasan morfologi sel sehingga mempermudah proses interpretasi. Sebaliknya, sediaan dengan kualitas pewarnaan yang kurang baik dapat menyulitkan pembacaan preparat. Methylene blue sebagai salah satu reagen pada metode pewarnaan Diff-Quick digunakan secara berulang dalam pemeriksaan rutin sehingga perlu diketahui pengaruh durasi pemakaiannya terhadap kualitas sediaan sitologi. Tujuan:Mengetahui pengaruh durasi pemakaian reagen methylene blue terhadap kualitas sediaan sitologi cairan pleura pada pewarnaan Diff-Quick. Metode: Penelitian ini merupakan penelitian kuantitatif dengan desain eksperimen menggunakan 36 sediaan sitologi cairan pleura yang dibagi menjadi tiga kelompok, yaitu running ke-1 (50 detik), running ke-5 (60 detik), dan running ke-10 (70 detik). Penilaian kualitas sediaan dilakukan berdasarkan kejelasan inti sel, kejelasan sitoplasma, kontras pewarnaan, dan kebersihan latar belakang. Data dianalisis menggunakan uji Kruskal–Wallis. Hasil: Rerata skor kualitas sediaan pada running ke-1 sebesar 8,75 ± 1,14, running ke-5 sebesar 10,00 ± 1,48, dan running ke-10 sebesar 10,08 ± 1,38. Hasil uji Kruskal–Wallis menunjukkan bahwa tidak terdapat perbedaan yang bermakna kualitas sediaan sitologi cairan pleura berdasarkan durasi pemakaian reagen methylene blue (p = 0,053). Kesimpulan: Durasi pemakaian reagen methylene blue tidak berpengaruh secara bermakna terhadap kualitas sediaan sitologi cairan pleura pada pewarnaan Diff-Quick (p > 0,05), sehingga hipotesis alternatif ditolak. Reagen methylene blue masih mampu menghasilkan kualitas sediaan pada kategori cukup hingga baik sampai running ke-10, sehingga masih layak digunakan dalam pemeriksaan sitologi cairan pleura. Background: The quality of pleural fluid cytology slides plays an important role in supporting clinicians in establishing a diagnosis. Poor staining quality may obscure cellular morphology and make microscopic interpretation more difficult. Methylene blue is one of the reagents used in the Diff-Quick staining method, and repeated use of this reagent may affect staining quality. Objective: To determine the effect of the duration of methylene blue reagent use in Diff-Quick staining on the quality of pleural fluid cytology slides. Methods: This study employed a quantitative experimental design using a post-test only approach. A total of 36 pleural fluid cytology slides were divided into three groups based on the duration of methylene blue reagent use: 50 seconds (1st run), 60 seconds (5th run), and 70 seconds (10th run), with 12 slides in each group. Slide quality was assessed using a scoring system based on nuclear clarity, cytoplasmic clarity, staining contrast, and background cleanliness. Data were analyzed using the Kruskal–Wallis test with a significance level of 95% (α = 0.05). Results: The mean quality scores were 8.75 ± 1.14 for the 50-second group, 10.00 ± 1.48 for the 60-second group, and 10.08 ± 1.38 for the 70-second group. Statistical analysis using the Kruskal–Wallis test showed no significant difference in slide quality among the three groups (p = 0.053). Conclusion: The duration of methylene blue reagent use did not have a statistically significant effect on the quality of pleural fluid cytology slides stained using the Diff-Quick method (p > 0.05); therefore, the alternative hypothesis was rejected. However, the methylene blue reagent was still able to produce fair to good-quality cytology slides up to the 10th run, indicating that it remains suitable for use in pleural fluid cytology examinations.
