Abstract
ABSTRAK
Latar Belakang: Fraktur biasa disebut sebagai patah tulang yang biasanya disebabkan oleh trauma atau tekanan fisik. Patahan yang terjadi biasanya tidak hanya berasal dari satu retakan saja melainkan dari banyaknya retakan, umumnya patahan terjadi secara lengkap dan fragmen pada tulangnya bergeser. Berdasarkan sifat fraktur dibagi menjadi dua yaitu fraktur tertutup dan fraktur terbuka. Fraktur tertutup umumnya terjadi saat patahan tulang tidak menembus kulit, sedangkan fraktur terbuka patahan yang terjadi menembus kulit. Fraktur terbuka yang berisiko tinggi menyebabkan perdarahan masif sehingga dapat menimbulkan hipovolemia. Kondisi ini memerlukan penanganan keperawatan yang cepat dan tepat untuk mencegah terjadinya syok dan komplikasi lanjut. Tujuan: Menjelaskan implementasi keperawatan manajemen hipovolemia pada pasien dengan fraktur terbuka di Instalasi Gawat Darurat (IGD) RS Bhayangkara Palembang tahun 2026. Metode: Studi kasus dilakukan terhadap pasien fraktur terbuka dengan masalah keperawatan hipovolemia. Intervensi keperawatan meliputi observasi tanda dan gejala hipovolemia, pemantauan cairan masuk dan keluar, pemberian cairan intravena, posisi terapeutik, edukasi asupan cairan oral, serta kolaborasi dalam pemberian cairan isotonik dan transfusi darah bila diperlukan. Hasil: Implementasi keperawatan manajemen hipovolemia menunjukkan peningkatan status hemodinamik pasien, ditandai dengan stabilnya tanda vital dan keseimbangan cairan tubuh. Kesimpulan: Manajemen keperawatan yang komprehensif dan tepat waktu efektif dalam menstabilkan kondisi pasien fraktur terbuka dengan hipovolemia, serta mencegah komplikasi lebih lanjut. Diperlukan keterampilan klinis yang baik dari perawat dalam melakukan identifikasi dan penanganan kondisi hipovolemia secara cepat dan akurat.
ABSTRACT
Background: Fractures are commonly referred to as broken bones which are usually caused by trauma or physical pressure. The fractures that occur usually do not only come from one crack but from many cracks, generally the fracture occurs completely and the fragments in the bone are displaced. Based on the nature of the fracture is divided into two, namely closed fractures and open fractures. Closed fractures generally occur when the bone fracture does not penetrate the skin, while open fractures occur when the fracture penetrates the skin. High-risk open fractures cause massive bleeding which can cause hypovolemia. This condition requires fast and appropriate nursing care to prevent shock and further complications. Objective: To explain the implementation of nursing management of hypovolemia in patients with open fractures in the Emergency Department (IGD) of Bhayangkara Hospital Palembang in 2026. Method: A case study was conducted on an open fracture patient with hypovolemia nursing problems. Nursing interventions include observing signs and symptoms of hypovolemia, monitoring fluid intake and output, administering intravenous fluids, therapeutic positioning, education on oral fluid intake, and collaboration in administering isotonic fluids and blood transfusions if needed. Results: Implementation of hypovolemia nursing management demonstrated improved hemodynamic status, as evidenced by stable vital signs and fluid balance. Conclusion: Comprehensive and timely nursing management is effective in stabilizing patients with open fractures with hypovolemia and preventing further complications. Nurses require strong clinical skills to identify and manage hypovolemia quickly and accurately.
