ASUHAN KEBIDANAN KOMPREHENSIF PADA NY."D" DI TEMPAT PRAKTIK MANDIRI BIDAN WIWIET WULANDARI KOTA PALEMBANG

Latar Belakang
Masa kehamilan, persalinan, nifas, dan neonatus merupakan keadaan fisiologis yang berisiko mengancam jiwa ibu dan bayi, bahkan dapat menyebabkan kematian. Salah satu upaya yang dapat dilakukan oleh bidan adalah dengan menerapkan model asuhan kebidanan yang komprehensif dan berkelanjutan (Continuity of Care). Asuhan kebidanan komprehensif mencakup pemeriksaan yang rinci, menyeluruh, dan berkesinambungan pada ibu hamil, bersalin, nifas, serta pada bayi baru lahir. Penerapan asuhan kebidanan yang komprehensif dapat mengoptimalkan deteksi risiko tinggi maternal neonatal.
Tujuan
Tujuan dari penulisan laporan ini adalah untuk memberikan asuhan kebidanan komprehensif dengan pendekatan manajemen kebidanan dan pendokumentasian dalam bentuk SOAP pada Ny. "D" di Tempat Praktik Mandiri Bidan Wiwiet Wulandari Kota Palembang.
Metode
Penúlisan laporan tugas akhir ini menggunakan metode studi kasus dengan sasaran Ny. "D" umur 33 tahun hamil anak pertama. Teknik pengumpulan data yang digunakan adalah observasi, wawancara, dan dokumentasi.
Hasil
Asuhan kebidanan komprehensif pada Ny. "D" G1P0A0 persalinan normal pada usia kehamilan 39 minggu 4 hari, bayi lahir pada tanggal 05 Mei 2026 pukul 23.37 WIB, berat bayi 3.600 gram, panjang badan 48 cm, menangis kuat, kulit kemerahan, bergerak aktif, dan tidak ada kelainan.
Kesimpulan
Kesimpulan dari laporan ini yaitu mahasiswa mampu melakukan pengkajian data subjektif, objektif, menegakkan diagnosa, analisa, dan penatalaksanaan serta melaksanakan asuhan kebidanan komprehensif pada Ny. "D" di Tempat Praktik Mandiri Bidan Wiwiet Wulandari Kota Palembang Tahun 2026. Melalui penelitian ini diharapkan agar penelitian selanjutnya dikembangkan lebih lanjut dengan melakukan pemeriksaan yang lebih lengkap serta meningkatkan kualitas dan mutu pelayanan kesehatan sesuai standar pelayanan yang diterapkan.

The background
Pregnancy, childbirth, postpartum, and neonatal periods are physiological conditions that pose a risk to the lives of the mother and baby, and can even cause death. One of the efforts that can be made by midwives is to implement a comprehensive and continuous midwifery care model (Continuity of Care). Comprehensive midwifery care includes detailed, thorough, and continuous examinations of pregnant women, those giving birth, those in the postpartum period, and newborns. The implementation of comprehensive midwifery care can optimize the detection of high maternal neonatal risk.
The purpose
The purpose of this report is to provide comprehensive midwifery care with a midwifery management approach and documentation in SOAP format for Mrs. "D" at the Independent Practice of Midwife Wiwiet Wulandari in Palembang City.
The method of writing this final assignment report uses a case study method with the target being Mrs. "D" aged 33 years pregnant with her first child. The data collection techniques used are observation, interviews, and documentation.
The results
comprehensive midwifery care for Mrs. "D" G1P0A0 normal delivery at 39 weeks of gestation, the baby was born on May 05, 2026 at 23.37 WIB, the baby weighed 3,600 grams, was 48 cm long, cried loudly, had reddish skin, moved actively, and had no abnormalities.
The conclusion
The conclusion of this report is that students are able to conduct subjective and objective data assessments, establish diagnoses, analyze, and manage, and carry out comprehensive midwifery care for Mrs. "D" at the Independent Practice of Midwife Wiwiet Wulandari, Palembang City in 2026. Through this research, it is hoped that further research will be developed further by conducting more complete examinations and improving the quality and quality of health services according to the applied service standards.
