Latar Belakang : Asuhan kebidanan komprehensif merupakan pelayanan
berkelanjutan yang diberikan kepada ibu mulai masa kehamilan, persalinan, nifas,
bayi baru lahir, hingga pemilihan kontrasepsi guna mendeteksi secara dini adanya
komplikasi serta meningkatkan kesehatan ibu dan bayi. Laporan tugas akhir ini
bertujuan untuk memberikan asuhan kebidanan komprehensif pada Ny. R umur 22
tahun G1P0A0 di Klinik Klinik Aulia Khalik Medika Tahun 2026 dengan
pendekatan manajemen kebidanan dan pendokumentasian SOAP. Tujuan : Dari
penulisan laporan ini Adalah untuk memberikan asuhan kebidanan komprehensif
dengan pendekatan manajemen kebidanan dan pendokumentasian pada Ny.”R”
Klinik Aulia Khalik Medika kota Palembang. Metode : yang digunakan adalah
studi kasus deskriptif dengan pendekatan continuity of care yang dilaksanakan
mulai tanggal 28 Februari 2026 sampai 11 Mei 2026. Asuhan diberikan pada masa
kehamilan trimester III, persalinan, nifas, dan bayi baru lahir melalui pengkajian,
pemeriksaan fisik, observasi, konseling, pendidikan kesehatan, serta
penatalaksanaan sesuai standar pelayanan kebidanan. Hasil : Hasil asuhan pada
masa kehamilan menunjukkan ibu mengalami ketidaknyamanan fisiologis berupa
nyeri pinggang dan gangguan tidur pada trimester III yang dapat diatasi melalui
edukasi posisi tidur, istirahat cukup, latihan peregangan, dan dukungan psikologis.
Persalinan berlangsung normal pada usia kehamilan 40 minggu 2 hari dengan
persalinan spontan pervaginam, bayi lahir perempuan pukul 05.00 WIB, menangis
kuat, gerakan aktif, berat badan lahir 32000 gram, panjang badan 47 cm, dan tidak
ditemukan komplikasi pada ibu maupun bayi. Masa nifas berlangsung normal
dengan involusi uterus baik, pengeluaran ASI lancar, serta luka jahitan perineum
derajat II sembuh dengan baik. Pada masa neonatus, bayi dalam keadaan sehat,
refleks baik, tidak ditemukan tanda infeksi maupun komplikasi, serta mendapatkan
ASI eksklusif. Kesimpulan : dari asuhan kebidanan komprehensif pada Ny. R
menunjukkan bahwa kehamilan, persalinan, nifas, dan bayi baru lahir berlangsung
fisiologis tanpa komplikasi. Diharapkan bidan dapat terus meningkatkan pelayanan
continuity of care melalui deteksi dini, edukasi, dan pendampingan secara
berkesinambungan untuk meningkatkan kesehatan ibu dan bayi.

Background: Comprehensive obstetric care is a continuous service provided to
mothers from pregnancy, childbirth, postpartum and newborns, to the selection of
contraceptives to detect complications early and improve the health of the mother
and baby. This final project report aims to provide comprehensive midwifery care
to Mrs. R, aged 22 years G1P0A0 at the Aulia Khalik Medika Clinic in 2026 with
an approach to midwifery management and SOAP documentation. Objective: From
the writing of this report is to provide comprehensive obstetric care with an
approach to midwifery management and documentation to Mrs. R" Aulia Khalik
Medika Clinic in Palembang city. The method : : used is a descriptive case study
with a continuity of care approach which was carried out from February 28, 2026
to May 11, 2026. Care is provided during the third trimester of pregnancy,
childbirth, postpartum and newborns through examinations, physical
examinations, observations, counseling, health education, and management
according to midwifery service standards. Results s: The results of care during
pregnancy showed that the mother experienced physiological discomfort in the form
of low back pain and sleep disorders in the third trimester which could be overcome
through sleep position education, adequate rest, stretching exercises, and
psychological support. Childbirth took place normally at 40 weeks 2 days of
gestation with vaginal spontaneous delivery, the baby was born female at 05.00
WIB, cried strongly, active movements, birth weight 32000 grams, body length 47
cm, and no complications were found in the mother or baby. The postpartum period
takes place normally with good uterine involution, smooth milk production, and a
good second degree perineal suture wound. During the neonatal period, the baby
is in good health, has good reflexes, no signs of infection or complications are
found, and receives exclusive breast milk. Conclusion: from comprehensive
obstetric care on Mrs. R showed that pregnancy, childbirth, postpartum and
newborns took place physiologically without complications. It is hoped that
midwives can continue to improve continuity of care services through early
detection, education, and continuous assistance to improve maternal and infant
health.
