Abstract

Pendahuluan: Pemeriksaan kadar kalsium serum penting untuk diagnosis dan pemantauan terapi karena berperan dalam kontraksi otot, transmisi saraf, pembekuan darah, dan regulasi hormon. Mutu hasil sangat dipengaruhi jenis reagen. RSUP Dr. Mohammad Hoesin Palembang menggunakan reagen Cac pada alat Alinity, sementara tersedia reagen Cac2 Sigma Strong yang diklaim memiliki kinerja lebih baik, sehingga perlu dianalisis kesetaraan hasilnya. Metode: Penelitian analitik komparatif ini dilakukan pada Juni 2026 terhadap 175 sampel serum pasien yang diperiksa secara paralel
menggunakan reagen Cac dan Cac2 Sigma Strong di alat Alinity. Analisis data menggunakan statistik deskriptif dan uji Mann-Whitney karena data tidak berdistribusi normal. Hasil: Kedua reagen memenuhi standar mutu dengan %CV ≤ 3,108% dan %TEa ≤ 4,557%. Rata-rata kadar kalsium pada reagen Cac sebesar 8,593 mg/dL dan Cac2 Sigma Strong sebesar 8,612 mg/dL. Uji statistik menunjukkan p = 0,687 > 0,05. Kesimpulan: Tidak terdapat perbedaan signifikan secara statistik antara kedua reagen. Keduanya setara dan dapat saling dipertukarkan tanpa mengubah interpretasi klinis. Saran: Cac2 Sigma Strong dapat dijadikan alternatif pengganti Cac karena memiliki rentang ukur lebih luas, stabilitas urin lebih lama, dan menggunakan consolidate calibrator. Laboratorium tetap perlu menjalankan kontrol kualitas internal dan eksternal secara berkala.

Introduction: Serum calcium examination is essential for diagnosis and therapy monitoring due to its role in muscle contraction, nerve transmission, blood coagulation, and hormone regulation. Result quality is strongly influenced by reagent type. Dr. Mohammad Hoesin Hospital Palembang uses Cac reagent on the Alinity analyzer, while Cac2 Sigma Strong reagent is available and claimed to have better performance, thus requiring analysis of result equivalency. Methods: This comparative analytical study was conducted in June 2026 on 175 patient serum samples tested in parallel using Cac and Cac2 Sigma Strong reagents on the Alinity system. Data were analyzed using descriptive statistics and the Mann-Whitney test because data were not normally distributed. Results: Both reagents met quality standards with %CV ≤ 3.108% and %TEa ≤ 4.557%. Mean calcium level for Cac reagent was 8.593 mg/dL and for Cac2 Sigma Strong was 8.612 mg/dL. Statistical testing showed p = 0.687 > 0.05. Conclusion: There was no statistically significant difference between the two reagents. Both are equivalent and interchangeable without changing clinical interpretation. Recommendation: Cac2 Sigma Strong can be used as an alternative to Cac because it has a wider measurement range, longer urine stability, and uses a consolidate calibrator. Laboratories should continue performing internal and external quality control regularly.
