Abstract
Latar Belakang: Diabetes Melitus Tipe 2 adalah penyakit metabolik kronis yang ditandai dengan hiperglikemia akibat gangguan sekresi insulin, kerja insulin, atau keduanya. Hiperglikemia yang berkepanjangan dapat menyebabkan berbagai komplikasi mikrovaskular, termasuk nefropati diabetik. Kadar HbA1c digunakan sebagai indikator kontrol glikemik jangka panjang. HbA1c memiliki peran penting sebagai indikator dalam diagnosis dan manajemen Diabetes Melitus. Pemeriksaan HbA1c memiliki keunggulan dibandingkan pengukuran glukosa darah harian karena dapat memberikan gambaran umum kadar glukosa darah rata-rata selama 2–3 bulan terakhir, sedangkan kadar kreatinin digunakan sebagai indikator fungsi ginjal. Peningkatan kadar HbA1c diduga berhubungan dengan peningkatan kadar kreatinin sebagai akibat kerusakan ginjal pada pasien Diabetes Melitus Tipe 2. Tujuan: Untuk mengetahui korelasi antara kadar HbA1c dan kadar kreatinin pada pasien Diabetes Melitus Tipe 2 di RSUD Palembang BARI. Metode: Penelitian ini merupakan penelitian analitik kuantitatif dengan melakukan pengumpulan data yaitu data sekunder yang diperoleh dari rekam medis pasien Diabetes Melitus Tipe 2 yang menjalani pemeriksaan HbA1c dan kreatinin di laboratorium klinis RSUD Palembang BARI dari Mei 2025 hingga April 2026. Pengambilan sampel dilakukan menggunakan metode consecutive sampling dengan total 340 pasien. Pengujian normalitas dilakukan menggunakan uji Kolmogorov-Smirnov, dan analisis korelasi dilakukan menggunakan uji korelasi Spearman. Hasil: Uji korelasi Spearman menunjukkan nilai r = 0,308 dan nilai p = 0,000 (<0,05), menunjukkan korelasi positif dengan kekuatan sedang antara kadar HbA1c dan kadar kreatinin pada pasien Diabetes Melitus Tipe 2 di RSUD Palembang BARI. Kesimpulan: Terdapat hubungan yang signifikan secara statistik antara kadar HbA1c dan kadar kreatinin. Arah korelasinya positif (+), artinya semakin tinggi kadar HbA1c, semakin tinggi pula kecenderungan kadar kreatinin pada pasien Diabetes Melitus Tipe 2 di RSUD Palembang BARI.
Background: Type 2 diabetes mellitus is a chronic metabolic disease characterized by hyperglycemia due to impaired insulin secretion, insulin action, or both. Prolonged hyperglycemia can lead to various microvascular complications, including diabetic nephropathy. HbA1c levels are used as an indicator of long-term glycemic control. HbA1c plays an important role as an indicator in the diagnosis and management of diabetes mellitus. HbA1c testing has advantages over daily blood glucose measurements because it can provide an overview of average blood glucose levels over the past 2–3 months, while creatinine levels are used as an indicator of kidney function. Increased HbA1c levels are thought to be associated with increased creatinine levels as a result of kidney damage in patients with type 2 diabetes mellitus. Objective: To determine the correlation between HbA1c levels and creatinine levels in patients with type 2 diabetes mellitus at RSUD Palembang BARI. Methods: This study is a quantitative analytical study with an observational approach. Secondary data were obtained from medical records of type 2 diabetes mellitus patients who had HbA1c and creatinine examinations in the clinical laboratory of RSUD Palembang BARI from May 2025 to April 2026. Sampling was conducted using metode consecutive sampling with a total of 340 patient data. Normality testing was performed using the Kolmogorov-Smirnov test, and correlation analysis was conducted using the Spearman correlation test. Results: The Spearman correlation test showed a value of r = 0.308 and a p-value = 0.000 (<0.05), indicating a positive correlation with a moderate strength between HbA1c levels and creatinine levels in patients with type 2 diabetes mellitus at RSUD Palembang BARI. Conclusion: There is a statistically significant relationship between HbA1c levels and creatinine levels. The direction of the correlation is positive (+), meaning that the higher the HbA1c level, the higher the creatinine level tends to be in patients with type 2 diabetes mellitus at RSUD Palembang BARI.
