IMPLEMENTASI KEPERAWATAN PURSED LIP BREATHING DAN DIAPHRAGMATIC BREATHING PADA ANAK ASMA DENGAN GANGGUAN POLA NAPAS DI RSUD SITI FATIMAH PALEMBANG TAHUN 2026

ABSTRAK
Latar Belakang: Asma merupakan penyakit inflamasi kronis pada saluran napas yang sering terjadi pada anak dan dapat menyebabkan gangguan pola napas tidak efektif, ditandai dengan sesak napas, takipnea, penggunaan otot bantu pernapasan, serta penurunan saturasi oksigen. Salah satu upaya nonfarmakologis yang dapat dilakukan untuk memperbaiki kondisi tersebut adalah penerapan teknik Pursed Lip Breathing (PLB) dan Diaphragmatic Breathing (DB). Tujuan: Mendeskripsikan implementasi keperawatan Pursed Lip Breathing dan Diaphragmatic Breathing pada anak asma dengan gangguan pola napas serta menganalisis hasil penerapannya. Metode: Penelitian ini menggunakan metode studi kasus deskriptif dengan pendekatan asuhan keperawatan pada dua anak usia sekolah yang didiagnosis asma dengan masalah keperawatan pola napas tidak efektif di RSUD Siti Fatimah Palembang Tahun 2026. Implementasi dilakukan selama tiga hari dengan pemberian latihan Kombinasi Pursed Lip Breathing dan Diaphragmatic Breathing menggunakan media ballon blowing sebanyak tiga kali sehari, disertai pemantauan frekuensi napas, saturasi oksigen, serta tanda-tanda gangguan respirasi. Hasil: Setelah dilakukan implementasi, terjadi perbaikan pola napas pada kedua pasien. Pada pasien pertama frekuensi napas menurun dari 37 kali/menit menjadi 26 kali/menit dan saturasi oksigen meningkat dari 94% menjadi 99%, sedangkan pada pasien kedua frekuensi napas menurun dari 35 kali/menit menjadi 28 kali/menit dan saturasi oksigen meningkat dari 95% menjadi 99%. Penggunaan otot bantu napas, pernapasan cuping hidung, serta tanda sesak napas juga mengalami perbaikan. Kesimpulan: Implementasi Pursed Lip Breathing dengan ballon blowing dan Diaphragmatic Breathing efektif dalam memperbaiki pola napas, menurunkan frekuensi napas, serta meningkatkan saturasi oksigen pada anak dengan asma.

ABSTRACT
Background: Asthma is a chronic inflammatory disease of the airways that commonly occurs in children and may lead to an ineffective breathing pattern, characterized by dyspnea, tachypnea, the use of accessory respiratory muscles, and decreased oxygen saturation. One of the non-pharmacological interventions that can be applied to improve this condition is the use of Pursed Lip Breathing (PLB) and Diaphragmatic Breathing (DB) techniques. Objective: To describe the implementation of Pursed Lip Breathing and Diaphragmatic Breathing nursing interventions in children with asthma and ineffective breathing patterns and to analyze the outcomes of these interventions. Methods: This study employed a descriptive case study design using a nursing care approach involving two school-aged children diagnosed with asthma and an ineffective breathing pattern at Siti Fatimah Regional General Hospital Palembang in 2026. The intervention was conducted for three days through Pursed Lip Breathing exercises using balloon blowing media and Diaphragmatic Breathing exercises three times daily, accompanied by monitoring of respiratory rate, oxygen saturation, and signs of respiratory distress. Results: Following the intervention, improvements in breathing patterns were observed in both patients. In the first patient, the respiratory rate decreased from 37 breaths per minute to 26 breaths per minute, while oxygen saturation increased from 94% to 99%. In the second patient, the respiratory rate decreased from 35 breaths per minute to 28 breaths per minute, while oxygen saturation increased from 95% to 99%. Improvements were also noted in the use of accessory respiratory muscles, nasal flaring, and signs of dyspnea. Conclusion: The implementation of Pursed Lip Breathing combined with balloon blowing and Diaphragmatic Breathing was effective in improving breathing patterns, reducing respiratory rate, and increasing oxygen saturation in children with asthma.
