Abstract

Latar Belakang: Diabetes Melitus (DM) adalah penyakit kronis yang muncul ketika pankreas tidak menghasilkan cukup insulin atau ketika tubuh tidak mampu menggunakan insulin yang diproduksi secara efektif dan biasanya ditandai dengan hiperglikemia yaitu tingginya kadar glukosa dalam darah. Komplikasi diabetes melitus dapat dicegah dengan pemantauan kontrol glikemik yang baik. Kondisi hiperglikemia dalam jangka panjang yang ditunjukkan dengan peningkatan HbA1c, akan meningkatkan osmolaritas plasma dan menurunkan deformabilitas eritrosit. Perubahan ini dapat memengaruhi umur eritrosit, sehingga hasil HbA1c dapat berhubungan dengan profil eritrosit. Tujuan: Mengetahui korelasi antara HbA1c dengan profil eritrosit pada pasien diabetes melitus di RSUD Palembang BARI. Metode: Penelitian ini merupakan penelitian analitik kuantitatif retrospektif. Data sekunder diperoleh dari rekam medis pasien diabetes melitus yang melakukan pemeriksaan HbA1c, jumlah eritrosit, hemoglobin, hematokrit, dan indeks eritrosit di laboratorium klinik RSUD Palembang BARI pada periode Januari 2026 hingga Juni 2026. Pengambilan sampel dilakukan secara purposive sampling dengan jumlah 185 data pasien. Uji normalitas dilakukan dengan menggunakan Kolmogorov-Smirnov, dan analisis korelasi Spearman karena data tidak terdistribusi normal. Hasil: Hasil uji korelasi menunjukkan terdapat korelasi searah yang sangat lemah antara HbA1c dengan jumlah eritrosit (r = 0.189, p = 0.010), hemoglobin (r = 0.176, p = 0.017) dan hematokrit (r = 0.163, p = 0.027), serta korelasi berlawanan yang lemah antara HbA1c dengan MCV (r = -0.222, p = 0.002). Tidak terdapat korelasi antara HbA1c dengan MCH (p = 0.069) maupun MCHC (p = 0.371). Kesimpulan: Nilai HbA1c yang lebih tinggi cenderung berkorelasi dengan kenaikan jumlah eritrosit, hemoglobin, hematokrit dan penurunan MCV pada pasien diabetes melitus di RSUD Palembang BARI.

Background: Diabetes Mellitus (DM) is a chronic disease that occurs when the pancreas does not produce enough insulin or when the body cannot effectively use the insulin produced, and is typically characterized by hyperglycemia, which is high levels of glucose in blood. Diabetes mellitus complications can be prevented with good glycemic control monitoring. Long-term hyperglycemic conditions indicated by increased of HbA1c will increase plasma osmolarity and decrease erythrocyte deformability. These changes can affect erythrocyte lifespan, so HbA1c results may be related to erythrocyte profile. Objective: To determine the correlation between HbA1c with erythrocyte profile in diabetes mellitus patients at RSUD Palembang BARI. Methods: This study is a retrospective quantitative analytical study. Secondary data were obtained from medical records of diabetes mellitus patients who underwent HbA1c, erythrocyte count, hemoglobin, hematocrit, and erythrocyte indices examinations at the clinical laboratory of RSUD Palembang BARI during the period from January 2026 to June 2026. Sampling was conducted using purposive sampling with 185 patient data samples. Normality testing was performed using Kolmogorov-Smirnov, and relationship analysis using Spearman correlation test because the data was not normally distributed. Results: The correlation test results showed there was a vey weak positive relationship between HbA1c and erythrocyte count (r = 0.189, p = 0.010), hemoglobin (r = 0.176, p = 0.017) and hematocrit (r = 0.163, p = 0.027), and a weak negative relationship between HbA1c and MCV (r = -0.222, p = 0.002). There was no relationship between HbA1c with MCH (p = 0.069) and MCHC (p = 0.371). Conclusion: Higher HbA1c values tend to correlate with increased of erythrocyte count, hemoglobin, hematocrit, and decreased MCV in diabetes mellitus patients at RSUD Palembang BARI.
