ASUHAN KEBIDANAN KOMPREHENSIF PADA NY. “S” DI TEMPAT PRAKTIK MANDIRI BIDAN SORAYA KOTA PALEMBANG TAHUN 2026

Latar Belakang: Asuhan kebidanan Continuity of Care (CoC) merupakan pelayanan kebidanan berkesinambungan mulai dari kehamilan, persalinan, bayi baru lahir, nifas, hingga keluarga berencana guna meningkatkan kualitas kesehatan ibu dan bayi serta mendeteksi dini komplikasi. Laporan tugas akhir ini bertujuan memberikan asuhan kebidanan komprehensif pada Ny. “S” umur 33 tahun G7P6A0 di TPMB Soraya Kota Palembang Tahun 2026. Tujuan: Memberikan asuhan kebidanan secara komprehensif pada Ny. “S” umur 33 tahun G7P6A0 di TPMB Soraya Kota Palembang Tahun 2026. Metode: Penyusunan laporan tugas akhir ini menggunakan metode deskriptif dengan pendekatan studi kasus. Asuhan dilaksanakan mulai 06 Maret hingga 03 Mei 2026 meliputi kehamilan trimester III, persalinan, bayi baru lahir, masa nifas. Pengumpulan data dilakukan melalui wawancara, observasi, pemeriksaan fisik dan penunjang, serta dokumentasi menggunakan format SOAP. Hasil: Asuhan Kebidanan Komprehensif yang telah dilakukan pada Ny. “S” mengalami ketidaknyamanan trimester III berupa nyeri punggung bawah dan sulit tidur yang bersifat fisiologis dan dapat di atasi dengan edukasi, istirahat cukup, kompres hangat dan penerapan teknik self hypnosis. Persalinan berlangsung normal secara spontan pervaginam pada kehamilan 41minggu. Bayi lahir pada 24 April 2026 pukul 03.23 WIB, berjenis kelamin laki-laki, berat badan 3.000 gram, panjang badan 51 cm, Apgar Score 9/10, kondisi bayi baik. Pada masa nifas, involusi uterus berlangsung normal, ASI lancar, ibu mampu merawat bayi dengan baik, dan bayi baru lahir dalam kondisi sehat, serta tidak ditemukan komplikasi selama masa pemantauan. Kesimpulan: Penerapan asuhan kebidanan Continuity of Care pada Ny. “S” menunjukkan bahwa pelayanan kebidanan komprehensif yang berkesinambungan dapat menjaga kesehatan ibu dan bayi, khususnya pada kasus grandemultipara dengan risiko obstetri lebih tinggi, sehingga komplikasi dapat dideteksi dini dan ditangani secara cepat dan tepat.

Background: Continuity of Care (CoC) midwifery care is a continuous midwifery service from pregnancy, childbirth, newborn care, postpartum care, to family planning to improve the quality of maternal and infant health and detect complications early. This final project report aims to provide comprehensive midwifery care for Mrs. “S” aged 33 years G7P6A0 at TPMB Soraya, Palembang City in 2026. Objective: To provide comprehensive midwifery care to Mrs. “S” aged 33 years G7P6A0 at TPMB Soraya, Palembang City in 2026. Method: This final project report was prepared using a descriptive method with a case study approach. The care was carried out from March 6 to May 3, 2026, covering the third trimester of pregnancy, childbirth, newborns, and the postpartum period. Data collection was carried out through interviews, observations, physical and supporting examinations, and documentation using the SOAP format. Results: Comprehensive Midwifery Care that has been carried out on Mrs. “S” experienced third trimester discomfort in the form of lower back pain and difficulty sleeping which were physiological and could be overcome with education, adequate rest, warm compresses and the application of self-hypnosis techniques. Labor took place normally spontaneously pervaginally at 41 weeks of pregnancy. The baby was born
on April 24, 2026, at 3:23 a.m. WIB. He was a boy, weighing 3,000 grams, measuring 51 cm long, with an Apgar score of 9/10. He was in good condition. During the postpartum period, uterine involution was normal, breast milk flow was
smooth, the mother was able to care for the baby well, and the newborn was healthy. No complications were found during the monitoring period. Conclusion: The implementation of Continuity of Care midwifery care for Mrs. "S" demonstrates that continuous, comprehensive midwifery services can maintain the health of both mother and baby, especially in cases of grandemultipara with higher obstetric risks. This allows complications to be detected early and managed quickly and appropriately.
